2008 UDC Nationals Heart Clinic(All Breeds Welcome)Monday May 12th, Tuesday May 13th Conducted by board certified cardiologist Dr. Nancy Morris and Sue Krom, Director of Cardiology Services.
RegistrationAll appointments are on a first-registered and paid basis. Participants MUST pre-register here. Sue Krom will schedule appointments. Please list under comments on the registration whether you have a conflict with showing or prefer a certain day or time. We will accommodate as much as possible. Participants must PRE-PAY check made out to MVC and mailed to Sue Krom 8 Cranberry Dr. Holyoke, MA or MC/Visa application online and faxed to 781-826-7608 You will not be considered completely registered until they have received payment. Late entries and payments MAY be taken provided that the number limits for the clinic have not been reached. AppointmentsSue Krom will contact you via email or telephone to confirm the time of your appointment. Please make sure to provide COMPLETE registration information. CostYou may purchase any single test or all tests. Information collected can be used for the 2008 UDC Breed Survey Test. Doppler Echo: $175.00 reports given after appointment Holter: $135.00 Cardio Care Blood Test: $55.00 OFA Heart Registry paperwork: $7.50 must be completed in advance
